Why would his body look like he was flying into the window if he had fallen out of it? also the reflections don't look right to me.

but I would say Fake.

Although it may be possible if the guy had some sort of cavity from the eye socket into his nasal area due to either disease or an operation, the shape of his finger is all wrong, it would have to be double jointed to bend in that direction.

I agree with Horus about the finger and also thought the finger looked too long, but...having experimented - without sticking my finger up my nose in any way shape at all - it may just be possible.

I think that they are big, but not that big. It is just the closeness to the camera that makes then look a lot bigger than they really are.
